Surface Music Kit (SMK) is a music production app that makes creating music more intuitive for beginner musicians.

My goal was to design the quantization feature so that it would be more quick and intuitive to use when producing music.
Patients need a clear and quick digital way to complete intake forms provided by clinicians.
People don’t remember what meds they took for depression over what period of time.

Patients have to complete a 13 page paper intake form and email it to the clinician.

To reduce friction, my  goal was to streamline patient onboarding through an online intake form experience.

Explorations

Working closely with Hyka’s Chief Medical Officer and Producer Designers, I created a customizable intake form process for the clinician side and patient side.
Dashboard
The intake form dashboard allows the clinician to quickly see and respond to forms that have been completed by potential patients.

Information Architecture
An intake form section is added for clinicians to create new intake forms and keep track of pending forms.
Pending Forms
The pending form page displays the list of intake forms waiting to be completed by patients.

Clinicians can choose to follow up or cancel invitations to patients.
Templates
Clinicians can build their own intake forms for patients to complete.

This allows the process to be streamlined and scalable for emerging treatment practitioners and more complex hospital systems.
